Our office maintains birth and death
records for people who were born or who died in Defiance County
between December 1908 to present.
Certified copies of birth
and death records can be purchased for a fee of $25.00 each.
Defiance County now
has statewide access to all Ohio birth records - December 1908 to
present.

- Need to complete a request form and provide payment. We
accept cash or checks
with a valid I.D.
(sorry, we cannot accept out-of-state personal checks).
Until further notice, we are
unable to accept debit/credit cards). Birth/Death certificates are issued in
the office Monday through Friday, between 8:30 and 4:15 - usually within a few minutes. |

BY MAIL -
Download a request form
for either a
Birth or
Death
certificate. Complete the request form and include the signature of the
person who is purchasing the certificate. Mail the completed request
form along with a check (sorry, we cannot accept out-of-state
personal checks), or money order and a self-addressed,
stamped business-size envelope. Mailed requests are usually
processed the same day they are received. Please use the mailing
address from above. |

- Go to
VitalChek
and place your order. Orders must be placed by 3:00 p.m.
 EST in order for our office to process the same day.
If you encounter any problems with the on-line order form, please
contact
VitalChek
at 1-800-669-8312. When
discussing your order with
VitalChek, please make sure your
order does not get routed through the state office (Ohio Department
of Health, Office of Vital Statistics, Columbus, OH). Orders routed
through the state office take much longer to process than orders
processed through our office.
