Strategic Plan 

The strategic planning process was completed over the course of three (3) sessions. Defiance
County General Health District (DCGHD) used input from a cross section of DCGHD department
members to determine the strategic direction for the agency.

Defiance County General Health District continues work towards the vision of “A Healthy and Safe Defiance County”. With a mission of promoting and protecting the health and well-being of all people and places in Defiance County; DCGHD focuses on our core values of Educating, Collaboration, Empowering, Respect, and Making a Difference. Annual reports summarize the work of the Health District throughout the year. Annual reports are presented to and approved by the Board of Health and District Advisory Council annually.

Defiance County Community Health Assessment

The Defiance County Health Assessment (CHA) encompasses health beliefs, perceptions and practices of residents of Defiance County to take a close look at the health of the residents as well as the issues that affect their health status. The Community Health Improvement Plan (CHIP) was developed by POWER Defiance County using information provided in the CHA.
